What Happens After I Contact You?
A Healthy Moms and Babes team member will follow up to explain the program, discuss your needs, eligibility and complete and submit an intake referral.
After your referral is submitted, you will be added to our list to be assigned to one of our trained and dedicated Home Visitors who will then contact you to discuss your availability to meet and receive services where your journey with Heallthy Moms and Babes will begin.
